1. Acceptance of Terms

These Terms of Service ("Terms") govern your access to and use of the Glam Medspa website and services. By visiting our website, submitting a contact form, or receiving services at our practice, you agree to these Terms in full. Glam Medspa reserves the right to update these Terms at any time, with changes effective upon posting to this page.

2. Services Description

Glam Medspa offers physician-supervised medical aesthetic treatments including but not limited to:

• Botox and Neurotoxin Injections
• Hyaluronic Acid Dermal Fillers
• Laser Skin Resurfacing and Rejuvenation
• Medical Grade Facial Treatments
• PDO Thread Lifts
• Sclerotherapy (Spider Vein Treatment)
• Non-Surgical Body Contouring

All services are performed or directly supervised by Dr. Quan Nguyen, MD, or licensed clinical staff under physician oversight. Services are provided by appointment only.

4. Appointments and Cancellation Policy

Scheduling

Appointments may be scheduled by calling (682) 493-0947 or through our online contact form. All appointments are subject to availability and practitioner discretion.

Cancellations and No-Shows

We require a minimum of 24 hours notice for appointment cancellations or rescheduling. Late cancellations or no-shows may result in a cancellation fee. Repeated no-shows may result in termination of the provider-patient relationship. Emergency exceptions are handled on a case-by-case basis.

Late Arrivals

Patients arriving more than 15 minutes late may need to be rescheduled to ensure adequate time for safe and effective treatment.

5. Payment and Billing

Payment is due at the time of service unless prior arrangements have been made. We accept major credit cards and other payment methods as communicated at time of booking. Prices for services are subject to change. Aesthetic services are generally not covered by insurance and are the financial responsibility of the patient. We do not accept insurance for elective cosmetic procedures.

Deposits may be required for certain appointments or package bookings and are non-refundable unless the appointment is cancelled with adequate notice per our cancellation policy.

6. Treatment Results and Expectations

Individual results from aesthetic treatments vary significantly based on factors including age, skin type, lifestyle, medical history, and adherence to post-treatment instructions. Glam Medspa makes no guarantee of specific outcomes. Before-and-after images shown in marketing materials represent individual patient results and are not guarantees of what you will experience.

You will receive a thorough consultation prior to any treatment to discuss realistic expectations, potential risks, and alternatives.

7. Patient Responsibilities

As a patient or prospective patient, you agree to:

• Provide complete and accurate medical history and health information
• Disclose all current medications, supplements, and allergies
• Follow all pre- and post-treatment instructions provided by our staff
• Attend all recommended follow-up appointments
• Notify us immediately of any adverse reactions or concerns
• Be at least 18 years of age, or have written parental/guardian consent if applicable

Nothing in these Terms limits liability for gross negligence, willful misconduct, or any liability that cannot be excluded by law.
